NEW Launch of online staff COVID reporting form

Quick details:

  • Staff who are affected* by COVID-19 are asked to submit their information via this online form instead of dialing ext. 1919.
  • If you have already contacted OHS via voicemail at ext. 1919 or sent an email, please do not fill out a form unless there has been a change in your status. We are diligently working through the backlog of calls.
  • The online form and QR code is shared below and can be accessed at work and at home.
  • Upon submission of the online form, staff members will receive information on up-to-date GRH protocols regarding isolation, testing and return to work that are specific to their reported COVID-19 situation.
  • The Occupational Health and Safety (OHS) Team will review the information provided, and in most cases, will contact the staff member to discuss their situation and evaluate their eligibility to return to work. 
  • This process supports managers and CSO by more efficiently monitoring when staff are able to return to work, in turn facilitating patient care.

The OHS and IT teams have developed an online form for staff to report their COVID-19 information. After filling out the online form, staff members will receive direction regarding the most appropriate next steps, tailored to the information they provided.

The information received will be reviewed by members of the OHS team. Most staff will receive a phone call to discuss their situation and develop a specific RTW plan, which will then be communicated to the manager and CSO.

Extension 1919 will remain operational, but staff are asked to transition to the online reporting format.
